ABSTRACT OF JUDGMENT
A judgment creditor (person who won a case) can record a judgment as a lien against a property. The judgment creditor fills out this form and submits it to the court. Once he has a certified copy of it, he records it with the Clark County Recorder's Office.

AFFIDAVIT OF RENEWAL OF JUDGMENT
A judgment creditor (person who won a case) can use this form if she wants to renew the judgment. This form must be filed at least 90 days before the expiration of a judgment.

AMENDED COMPLAINT 
An amended complaint form can be used when a plaintiff wants to make a change to his complaint. These changes can include adding or removing a party to the case or changing the amount of the claim. The filed amended complaint must be served on the defendant(s).

APPEAL PACKET
If you are appealing your case to the Eighth Judicial District Court, fill out this packet and file with the Las Vegas Justice Court. You will be required to either pay a $250.00 bond or the entire amount of the judgment against you, unless the court ordered otherwise. A bond cannot be waived.

CLAIM OF EXEMPTION FROM EXECUTION
This form can be used when a judgment debtor (person who owes money) or a third-party is being executed on, and believes the property that might be taken is exempt under Nevada law. This form must be filed within 10 days of when the notice of attachment or garnishment was mailed. After this form is filed, it must be mailed to the sheriff or constable, the judgment creditor (person owed the money), and the garnishee (usually the bank or employer).

COMPLAINT 
The small claims complaint form is filed with the court along with a copy of the mailed demand letter and proof that the demand letter was mailed via certified mail. These are the only documents required and filed to open a case. The complaint must be served on the defendant after a trial date has been entered on the summons at the bottom of the complaint.

COUNTERCLAIM 
If a defendant wants to sue the plaintiff, this counterclaim form must be filed and a $71.00 fee will be charged.  The counterclaim must be served on the plaintiff/counterdefendant after the counterclaim has been filed.

MOTION FOR SERVICE OF SMALL CLAIMS COMPLAINT OR COUNTERCLAIM BY CERTIFIED MAIL

This document has several steps and must be completed in the following order: 
Step 1: You must have a Declaration or Affidavit of Due Diligence completed and signed by the party who tried to serve the defendant/counterdefendant. If you used the sheriff or constable, you should have received an Affidavit of Due Diligence from either the sheriff or constable. However, if you used a private process server or friend, you can find a Declararation of Due Diligence form on this page for the process server or friend to complete and sign. File the completed Declaration or Affidavit with the court.
Step 2: Fill out and file the first page of the packet, titled Motion for Service of Small Claims Complaint by Certified Mail.
Step 3: Wait and see if the judge grants your motion. You will receive an Order for Service of Small Claims Complaint by Certified Mail if the judge grants your motion. If the judge denies your motion, you cannot continue.
Step 4:  Serve the complaint or counterclaim by certified mail, return receipt requested.
Step 5: Fill out and file the second page of the packet titled Certificate of Mailing. If the postal card is not returned with a date and signature, use a printout from the United States Postal Service's website showing the certified mail delivery being made. The day the mail was delivered is considered the date of service.

SUBPOENA
If you are requesting the appearance of a witness or a witness to bring documents to your trial, fill out this form and bring it to the Las Vegas Justice Court clerk's office for issuing. Once the deputy clerk signs it, you will have to make sure it is properly served on the party along with a check for a witness fee ($25 a day plus $0.58 per mile for a round trip drive to the court).
